The standard 4th Edition booklet exists with a dashed line on the cover leaf inside and without. I'd be interested whether this correlates with the different pricings.

I don't believe that there are MMQ cards made in the USA. I have bought various times cards from the US and all were looking like Belgium cards. Whoever reported this should double check his boosters.

You might be able to rule some sheets out by looking at the rarity breakdowns and calculate if there was space for a filler or not. Maybe you already did. Beside 4th Edition and Ice Age, there are Revised cards, but they are extremly rare and no lands are known if I remember right. One member here has claimed to own 8th Edition cards, but he didn't post scans.16) Which sets are know to have Alpha Cut corners?

1-2: I agree that similar misprints do exist from Revised (and Fallen Empires and Chronicles), but the pictured card is with high probability from the guy who non-stop kills uncut sheets and sells them on you know where.
